Send us the following information and we will send you an application form and copy of our constitution for your perusal. Please tell us:
- who referred you to our network or how you heard about us
- what your area of specialist support is for mothers or babies
- the geographical area you work in/around
- a brief introduction of yourself
On approval of your application we would like a copy of your qualifications and insurance where appropriate.
